Service Definitions

Vocational Services
  • Work Activity Training: jobs are provided to assist individuals in learning skills for a work setting. Also provided is specific training for individuals that are in need of a more direct and individualized type of training.

Residential Services

  • Supported living and Group living: Supported living is available to those individuals that want to live on their own in the community with minimal assistance. Group living is designed for those who want staff available to them for assistance and support.

Medical Services

  • Nursing Staff: each agency workshop is staffed with a RN and Medical Assistants to assist consumers with medical issues and scheduling appointments.

  • Medication Delegates: designated staff which has received additional training to assist consumers in monitoring daily medication.

  • Dental, Optical, Specialty Fields: the consumer may select local practitioners or out of town specialists. Staff will accompany the consumer as requested.
Financial Services
  • Payee Services: COF will provide payee services to individuals upon request.

  • Money Management: COF will assist consumers in coordinating their financial obligations and income.
Case Management Service
  • Case Management Services: coordinate supports, grievances, advocacy and crisis intervention services.

  • Case Manager or Service Coordinator: consumers will have a Service Coordinator of their choice.
